Buttock augmentation, or gluteal augmentation, is used to improve the contour, size and/or shape of the buttocks. This is done through the use of buttock implants, fat grafting or sometimes a combination of the two.

Buttock implants are silicone-filled devices that are surgically placed deep within the tissues of the buttock. Gluteal augmentation through the use of fat grafting involves the transfer of fat from one area of the body into the tissues of the buttocks. This technique is sometimes referred to as a Brazilian butt lift.

In addition to gluteal augmentation procedures, a buttock lift can be accomplished by the surgical removal of soft tissue.

Your buttock augmentation surgery can be achieved through either the use of implants or through transferring your existing fat or sometimes a combination of both. Your surgeon may choose between a variety of incision patterns and techniques.

The appropriate technique for you will be determined based on:

-Buttock size and shape

-The amount of fat you have on your body that is available for use through liposuction

-Your goals and activity level

-Skin quality and elasticity as well as the amount of extra skin

Medications are administered for your comfort during the surgical procedures.  The choices include intravenous sedation and general anesthesia.  Your doctor will recommend the best choice for you.  No matter which type of buttock augmentation procedure you have, anesthesia will be used to ensure your comfort during the procedure.

Buttock Implant Procedure

Step 1- Buttock implant procedure

An incision will be made.  The incision can be made toward the top on both sides of the upper buttock, toward the bottom in both of the gluteal creases (the areas where your “cheeks” meets the top of the thighs), incisions between both cheeks (hidden in the crease) or with one incision placed down the center of the sacrum, or the crease between the buttocks.  The length of the incision and scar is directly related to the affected areas.

Step 2- Implant placement

A silicone implant specially made for the buttocks is inserted either in the  muscle or above the gluteal muscle on each side.

The method for inserting and positioning implants depends on the type of implant, degree of enlargement desired, your body type, and your surgeon’s recommendations.

Step 3- Closing the incisions

Incisions are closed with layered sutures in the buttocks tissue and with sutures, skin adhesive or surgical tape to close skin.

Step 4- See the results

The results of the buttock augmentation are immediately visible.  Over time, post-surgical swelling will resolve and incision lines will fade.  Satisfaction with your new image should continue to grow as you recover and realize the fulfillment of your goal for fuller buttock.  Results of this surgery are permanent.

Fat Grafting/Brazilian Butt Lift Procedure

Step 1- Liposuction

Liposuction is performed through small, inconspicuous incisions.  First, diluted local anesthesia is infused to reduce bleeding and trauma.  Then a thin hollow tube, or cannula, is inserted through the incision to loosen excess fat using a controlled vacuum or syringe attached to the cannula.  In general, your surgeon may liposuction up to three times the amount of fat than what will actually be injected into the buttocks.

Step 2- Fat injection

The fat, tissue and blood that were removed through liposuction are then processed.  The syringes are then connected to a special cannula.  Your surgeon will use this cannula to add the fat to the buttocks through small injections all over your buttocks.

Step 3- See the results

The results of buttock augmentation are immediately visible; however you will not see your final results until a year after the surgery, as it takes a while for the fat to “take”.  This procedure offers natural looking and feeling results, but the projection of the buttocks may decrease over time.  Some of the initial volume you experience after your fat grafting procedure will fade as the fat is absorbed by your body (on average patients will retain 60-80% of the transferred fat, in some cases more in others less).  Satisfaction with your goal for a fuller buttock.  As you gain or lose weight, your results may change as the transferred fat can grow or shrink just as any other fat in your body.

ANESTHESIA

General. You will sleep through the entire procedure.

This surgery is performed on an outpatient basis.

RECUPERATION AND HEALING

Initial discomfort is controlled with oral pain medications.

Minimal swelling and bruising. However, swelling is more likely the larger the implant. This will disappear in time. Be patient.

When getting buttock augmentation with implants, two drains are placed during surgery. Typically, the first drain is removed 2-3 days after the surgery and the second drain at 7-10 days. Detailed drain care and a log sheet will be provided to you before surgery.

When getting implants the initial dressing on the incision is removed only by Dr. Hainer or a staff member in 1-3 days. It is crucial that you wear the specially designed garment that is provided for you to help in contouring the tissues and supporting the implants while the incisions heal. This is worn day and night for the first 2 weeks. After the first 2-week period, another garment is transitioned into and worn only during the day for an additional 4 weeks. Showering may be resumed once the initial dressing is changed, but tub bathing, whirlpools and swimming pools are prohibited for 4 weeks.

Some sutures will dissolve on their own. Some will be removed by Dr. Hainer or a staff member.

3 weeks off is recommended when getting buttock augmentation with implants. 1 week when getting fat grafting.

Strenuous exercise is to be avoided for 6 weeks when receiving buttock implants. With fat grafting, light activities can be resumed in 1 week and more strenuous exercise can resume in 2-3 weeks as long as lycra or spandex-type garment is worn. By 6 weeks, all activities can be resumed.

If any scar is exposed while outdoors, you should protect it with sunscreen SPF 15 or higher for at least on year to prevent permanent discoloration.

Should be able to feel comfortable in about 2 weeks, depending on swelling.

When receiving implants, it may take about 6 weeks for the buttock to regain its natural appearance, softness, and shape. With fat grafting final results are usually seen after 6 months.

Special Care

ABSOLUTELY NO SMOKING IF DONE WITH IMPLANTS!!!!!! No smoking for 4 weeks prior to surgery and at least 4 weeks after. If done with fat grafting, transporting home and to the office for your follow up visit, you should either be on your stomach or on your side. Because direct pressure will destroy the transferred fat, the patient is asked to keep pressure off of the buttocks for at least 6 weeks. Absolutely no sitting is permitted for 10 days (except for toileting). Then patients can sit with modifications. A pillow is placed behind the thighs, not touching but elevating the buttocks, effectively avoiding pressure. With this technique, patients can return to work, drive, and go to the movies at 10-14 days. Traditional sitting is not allowed for 6 weeks.
